When diagnostic imaging leads to intervention in the same session, whether the diagnostic study is separately billable depends on whether it was truly diagnostic or confirmatory. That determination requires reading the operative note, not applying a rule.
Selective catheterization coding
Catheter placement is coded by vascular family and order of selectivity. Miscount the order and the claim is wrong in a way that is difficult to catch downstream.
Overlapping global periods
Staged interventions, planned returns, and unrelated procedures during a global period each require different modifiers. Modifier 58, 78, and 79 are not interchangeable, and payers deny on the distinction.
Increased procedural services
Modifier 22 exists for cases that took substantially more work. It requires documentation that quantifies the additional effort, and most operative notes never make that case even when it is true.

Common Questions
Why do our angiography charges keep getting bundled?
Usually because the documentation does not establish the study was diagnostic rather than confirmatory. If a prior study already demonstrated the lesion, the imaging at intervention is generally not separately reportable. When it genuinely was diagnostic, the note has to say so.
Which modifier applies to a return to the operating room?
It depends on why. Modifier 58 for a staged or planned return, 78 for an unplanned return related to the original procedure, 79 for an unrelated procedure during the global period. Payers deny on the distinction and the appeal is harder than getting it right initially.
Is modifier 22 worth pursuing?
When the case genuinely required substantially more work, yes, but it requires documentation that quantifies it. Comparative language about time and difficulty against a typical case is what carries the appeal.
